After you have complete a volunteering session, you will need to fill this form out. Please fill the form out completely. To see how many hours you have accumulated over time, you can go to your students RenWeb account. The hours are labeled under “Service Hours”. Parents will nee 20 hours of Volunteer time per student, per year. We have many opportunities that you can stay aware of through our weekly newsletter The Falcon Times. We will also post many of our athletic season PIP opportunities on Sing-Up.com. Here is where you can see a general overview of what PIP is.
Our Transportation release Form is a document that gives your student permission to travel without taking our school provided form of transportation. For an example, this form would be filled out by a parent when their student athlete takes the team bust to an off campus game, but you prefer to then drive your student home from the game. Coaches and teachers will communicate to you when these are needed. We also keep copies in our school office for your convenience. We prefer to have this form in the office the day before the event. SUICIDE PREVENTION PARENTAL PERMISSION FORM
This permission form is for our Student Suicide Awareness and Prevention Workshop. Our next date is still to be determined. This workshop will be held in partnership with Douglas County Suicide Prevention Network. Area church youth leaders and pastors will also be present to provide emotional support.
